If you’d like to make a referral, please fill out our online Referral Form and complete one of our Admissions Forms. If you need more information on what your family member or patient will need to pack for a stay at our facility, review our Packing List for complete details on what to bring and what not to bring.
Requested for Referral & Admission:
- Psychiatric evaluation
- Psychological evaluation, including full-scale IQ score
- Discharge summaries or progress updates from previous placements or outpatient therapy
- Social history
- Psychosexual evaluation or risk assessment (if available)
- Information about sexually inappropriate behaviors, per client

About Our Admissions Process
Our admissions team is ready to help you navigate your questions or concerns about Alabama Clinical Schools. We’re available to review any information you have about a potential client who needs treatment and help walk them through the process. Our facility accepts some commercial plans and Medicaid, but if you’re concerned about paying for treatment, give us a call.
